BECOMING A PATIENT

New patients looking to schedule an appointment will need to first fill out and submit the Intake Form. We will review whether we're credentialed with your insurance plan and call you to coordinate an intake appointment.

  • Due to high volumes, it may take up to 5 business days to receive a return call

  • Intake appointments are in-person only. We do offer telemedicine services for follow-up appointments

  • We do not provide formal ADHD testing. Please provide us with documentation from childhood or adulthood neuropsychological evaluation if seeking treatment with controlled medications.

We do not accept Medicaid & Medicaid HMOs, Carelon Health, Independent Health Essential plans and MediSource. Empire Plan, Cigna, Fidelis, Kalos, MVP, MagnaCare, Meritan Health, Molina, and AARP primary.

Co-pays are to be paid at the time of your appointment. Co-pays for mental health coverage do vary from one carrier to another, review the back of your insurance card and contact your  carrier for questions regarding payment or authorizations.
